- Provide person-centred care coordination to all residents, adhering to our philosophy of care by promoting dignity, individual freedom, choice and lifestyle and meaningful quality living;
- create partnerships with residents and families to ensure their goals guide the planning and implementation of care; and
- work closely and supportively with the nursing team to plan and coordinate all care.
Job description:
- Conducts psychosocial assessment and coordinates/reviews assessments (including standardized assessments), evaluation of resident-centred goals and priorities and ensures needs are met. Makes referrals as appropriate;
- provides direct psychosocial care for clients/residents experiencing stressful situations such as end-of-life, bereavement, complex family situations and/or other life-altering events;
- recommends and develops appropriate care plans based on resident’s goals of care and assessments in collaboration with care teams. Provides oversight to ensure care planning and service delivery are proactive and flexible and acts as the primary point of contact for the resident, family or stakeholders;
- leads and oversees care conferences, rounds and care plan reviews; shares/reviews assessment findings with the care team and communicates resident’s care needs to members of the team;
- reviews all legal documents related to financial, personal, legal, health and end-of-life care (e.g. Advanced Care Directive, guardianship documentation, Enduring Power of Attorney and Wills). If any concerns are identified, recommends further steps to address any concerns;
- provides education for clients/residents and families related to various health condition, including dementia;
- documents in health records thoroughly in accordance with established standards, policies and procedures that meet regulatory requirements in a timely manner;
- maintains professional competency and continuing education in professional discipline and evaluating clinical practice in order to improve care through best practice policy, procedures and regulations; and
- provides learning opportunities for other staff and students including mentoring and coaching as required.

Job requirements:
- Bachelor of Social Work or Registered Nurse;
- experience providing case management;
- experience developing person-centred care plans;
- experience completing assessments in the respective field is an asset;
- TB screening;
- vulnerable sector RCMP security clearance;
- valid Class 5 driver’s license; and
- WHMIS 2015.
