Licensed practical nurses (LPN) provide person-centered care using best practice guidelines in a home-like environment, while respecting residents’ individuality.
LPNs use collaboration, teamwork and innovation daily as part of the interdisciplinary team, while growing professional skills.
Job description:
- Provides leadership in care to clients by promoting their physical, emotional and spiritual well-being according to the philosophy of care, policies, procedures and individual needs by:
o respecting individual choices and develop therapeutic relationships;
o assessing acute, chronic and palliative illnesses through observation, interview, palpation, inspection, auscultation, collection, discussion with the team and testing of specimens;
o developing care plans and case management strategies;
o providing nursing interventions according to care plans and evidence-based practice;
o administering prescribed medications and treatments;
o accepting, processing and verifying written/verbal/phone/faxed orders;
o demonstrating effective leadership, knowledge and skills in practice as well as in the management and supervision of other team members;
o assisting clients with the activities of daily living;
o assessing and evaluating client-centered goals; and
o documenting assessment, treatment and care activities on client health care records and sharing relevant information with other team members and disciplines. - Contributes to the development and effective functioning of the care team by:
o developing and maintaining harmonious relationships and managing conflict in an appropriate and timely manner; and
o adapting to changing environments through critical thinking and prioritizing competing demands and making decisions that will affect the health and safety of clients/staff. - Maintains professionalism through:
o continuous professional development and participation in quality improvement processes;
o ensuring annual completion of personal performance plan; and
o teaching, mentoring and coaching colleagues and student, as required.

Job requirements:
- Current Yukon LPN registration with Medication Administration upon hire or eligible for registration as a licensed practical nurse;
- TB screening;
- vulnerable sector RCMP security clearance;
- CPR level C with AED;
- Food Safe level 1 (completed within the last 2 years); and
- WHMIS 2015.
