Our social workers provide psychosocial assessments and supports in a range of settings and programs. They touch the lives of Yukoners at all stages of life, offering prevention education, counseling, support, care coordination and a variety of other services depending on the program and personal goals. Social workers are integral members of the following teams:  

Health services: 

  • Centre de Sante Constellation Health Centre – provides services to support the wellbeing of clinic patients including case management, health system navigation, referrals, education and counselling.
  • Counseling services – provides goal focused group and individual counseling for short- and long-term clients seeking support for substance use and/or mental wellness issues.
  • Forensic complex care team – works with clients in the criminal justice system and clients found not criminally responsible, providing trauma informed forensic assessments and counseling interventions. 
  • Long-term care – provides care coordination for Yukoners living in long-term care.
  • Palliative care – provides supports to people accessing end of life care.
  • Home care – offers care coordination and social work services for Yukoners aging in place or living at home with chronic health conditions.
  • Psychiatric outreach program/Early psychosis intervention – provides assessments, treatment plans, care coordination and case management for clients with serious and persistent mental illness.
  • Referred care clinic – works as part of an integrated care medical team offering harm-reduction and counseling supports to vulnerable individuals, often in an outreach context. 
  • Whitehorse walk-in clinic – offers advocacy, counselling and social work services in collaboration with health care and wellness partners in a medical setting across the age spectrum.
  • Withdrawal management services – working as part of an integrated care team, provides counseling, assessment, harm-reduction and case management supports to individuals looking to change their substance use patterns.

Social services: 

  • Adult protection – conducts risk assessments and case management for vulnerable adults who may be unable to seek assistance on their own.
  • Community outreach services – provides intensive case management for individuals who experience homelessness, live with substance use or mental health challenges, or have complex care needs and experience barriers to accessing care.
  • Family and children’s services – offers parenting supports, adoption services, child protection, and foster care in Whitehorse and the communities. 
  • Income support – provides assistance in the areas of income support, employment, crisis management and community living. 
  • Family and children’s services - Regional services – delivers child protection and other supports in Yukon’s diverse communities. 
  • Disability services– provides financial supports, services and referrals across the lifespan to children, youth and adults with disabilities.

Training:

Job requirements

The requirements differ depending on the nature of the social work role: 

  • Degree in social work (for most positions), or
  • degree in counselling, psychology, or social sciences (accepted for roles within mental wellness and substance use services);
  • to be either registered or eligible to be registered as a social worker in Canada (for roles in long-term care, palliative care and home care);
  • vulnerable sector RCMP security clearance;
  • WHMIS 2015 (within 1 month of hire); and 
  • TB screening (for roles in health settings).
Branch profile/spotlight

Health and Social Services (HSS) is the department that meets the needs of Yukon residents along the lifespan, from prenatal support though to services for seniors and end of life care. HSS programs include medical travel, insured health, community nursing, social supports, midwifery, chronic conditions supports, mental wellness and substance use services, emergency services, as well as other programs that support the health and wellbeing of all residents. HSS offers services via offices or clinics in Whitehorse and hubs in the many Yukon communities.
